Below is a sample nomination for the Ray T. Fortunato Award for Excellence in Human Resources.

This sample is meant to provide nominators with an idea of how much information and detail is recommended in the nomination process.

 
Criteria SAMPLE NOMINATION
 Pays proper attention to the "little things" James is extremely detail-oriented. During the initial implementation of the project, James took initiative in leading the weekly team meetings. He prepared weekly agendas, provided detailed minutes for every meeting, and updated the website with step-by-step instructions as HR processes were developed and further refined. James worked one-on-one with staff to ensure that everyone was trained and that work was completed consistently, accurately, and efficiently.
 Is visible to the people they serve

James has been a very visible and active member of our staff. Despite the fact that he has recently transitioned to our departmernt, he continues check on a regular basis to offer assistance and provide consultation to the staff; and, he continues to participate in weekly team meetings. In just a few short months, James has quickly become a "go to" person in our department as well. His door is always open when someone has a question.

Is a "can do" person with a positive attitude

James continues to take on additional work in our department while assisting with the project. He also serves on multiple workstream teams. Although his plate is quite full, he maintains a positive attitude and continues to ask if there is anything else he can do.

Demonstrates a sense of urgency

James is consistently reliable, and is quick to respond and provides follow-up whenever asked to research or respond to non-standard HR-related issues. All of his communications demonstrate a high degree of knowledge and professionalism.

Approaches work with a dedication to effectiveness and efficiency

James continues to work with the project team to evaluate the processes and recommend changes to improve efficiency and effectiveness; He is working closely with the others to assure continued alignment of processes as we move forward with Transformation.

Is inquisitive and strives to continually broaden their knowledge base

Due to his previous role as team lead, James was recently asked to provide temporary supervision of a consolidated team. Although he has never served in a supervisory capacity, James was more than willing to accept the assignment. Since that time, he has been completing supervisory training courses both online and face-to-face through CWLP. This demonstrates his commitment to excellence and professional growth.
