ACTIVE EMPLOYEES

Full-time Employee Health Plan 

  • Employees who have made a health care plan change or have newly elected a health care plan for 2021, WILL receive new ID cards from both Aetna and CVS Caremark, with an expected arrival date at their homes prior to January 1, 2021.  
  • Employees who have remained in the same health care plan from 2020 to 2021 will NOT receive new ID cards from Aetna or CVS Caremark, as the current cards are still active.  

Part-time Employee Health Plan  

  • Employees who have become eligible for health care benefits through the Affordable Care Act (ACA) as of January 1, 2021 WILL receive Aetna and CVS Caremark ID cards in mid-to-late January 2021, specific to the health care plan in which they enrolled. Coverage effective date is January 1, 2021 regardless of when the ID cards are received.
  • Eligible employees under ACA in 2020 and who are also eligible in 2021, will not receive new ID cards from Aetna and CVS Caremark, as the current cards are still active.  

Health Savings Accounts (HSA)

  • Employees newly enrolled for 2021 in the PPO Savings plan WILL receive Health Savings Account (HSA) Welcome Kits from HealthEquity at their homes, including a debit card, prior to January 1, 2021.  
  • Employees who are remaining in the PPO Savings plan for 2021 will NOT receive new debit cards from HealthEquity. The current cards will be replaced prior to their expiration date or report of loss/damage to the card.  

Flexible Spending Accounts (FSA)  

  • Employees newly enrolled in the Health Care Flexible Spending Account (FSA) will receive a Welcome Kit/Debit Card from HealthEquity at their homes; those enrolled in the Dependent Care FSA will receive a Welcome Kit from HealthEquity at their homes prior to January 1, 2021.    
  • Employees re-electing the Health Care FSA will NOT receive new debit cards. The current cards will be replaced prior to their expiration date or report of loss/damage to the card.  

Dental and/or Vision  

  • Employees newly enrolling in United Concordia Dental and/or EyeMed Vision plans WILL receive ID cards at their homes prior to January 1, 2021.  
  • Employees remaining in the Dental and/or Vision plans for 2021 will NOT receive new ID cards.  

RETIREES

  • Non-Medicare Eligible Retirees: For retirees who have made a health care plan change for 2021 (Retiree PPO plan or Retiree PPO Savings plan), new ID cards WILL be issued by both Aetna and CVS Caremark, with an expected arrival date at their homes prior to January 1, 2021.  
    • Non-Medicare Eligible Retirees who have remained in the same health care plan from 2020 to 2021 will NOT receive new ID cards from Aetna or CVS Caremark, as the current cards are still active.  
  • Medicare-Eligible Retirees:  Highmark Freedom Blue PPO will remain the health care plan for 2021. New ID cards will NOT be issued by Highmark, as the current cards are still active. New enrollees receive ID cards shortly after enrollment throughout the year.
